Solution:

Method 1:
Paste as a Word Table
Use this method to paste the Word table that contains form fields as a Word table in your new document:
1. In the original document, select the table with the form fields. To do this, move the insertion point into one of the table cells. On the Table menu, point to Select and click Table.
2. On the Edit menu, click Copy.
3. In your new Word document, click Paste Cells on the Edit menu.

Method 2:
Convert the Table to Text and Paste as an Object or Picture
Use this method to "temporarily" convert your Word table to text before you paste it as a Word Document object or picture:
1. In the original document, select the table with the form fields. To do this, move the insertion point into one of the table cells. On the Table menu, point to Select and click Table.
2. On the Edit menu, click Copy.
3. In your new Word document, click Paste Cells on the Edit menu.
4. With the table selected, point to Convert on the Table menu, and click Table to text.
5. In the Convert Table to Text dialog box, click Tabs and then click OK. The table is converted to text separated by tabs.
6. With the text and form fields selected, click Cut on the Edit menu.
7. On the Edit menu, click Paste Special.
8. In the Paste Special dialog box, click Microsoft Word Document Object or Picture and click OK.
